U.S. Care Management (USCM) delivers a unique 360° client-based model of care to improve the health of persons who suffer from some of the most debilitating chronic diseases.
The company's core service -- highly trained, community-based nurse care managers – allows USCM to more effectively coordinate care to complement all aspects of disease management programs. Our goal is to identify and engage members whose quality of life, severity of illness and health care costs can be improved through education, care coordination, resource identification and referral, coaching and guidance.
U.S. Care Management, previously Specialty Disease Management Services was organized in 1997 by health professionals who recognized the value in coordinating care to serve people with chronic diseases. Based in Florida, USCM provides services to government, public and private organizations across the country.
USCM is a wholly-owned subsidiary of U.S. Preventive Medicine, a Texas-based company leading the movement in preventive medicine.
