Our Stories

At U.S. Care Management we understand that social, economic, physical or psychological barriers often prevent people from complying with their prescribed medical care.  Our RN Care Managers work to overcome those barriers by providing the care, counsel, education and resources necessary for our members to successfully manage their condition. Here are a sample of our daily stories:



During a monitoring call, our RN Care Manager learned that an member was experiencing repeated episodes of nosebleeds during the week and was concerned about new medications prescribed for anxiety. The RNCM educated the member on his new medications, coached him on the importance of adherence and coached him on how to discuss his symptoms with his physician. The RNCM called the following day to learn he successfully contacted his physician and admitted to feeling less anxious with the reassurance.



During an initial assessment, a member reported that he had stopped his medications and had not seen his provider in three months, due to the lack of transportation. Our RN Care Manager contacted the provider to arrange a follow-up appointment and contacted the DHHS office to coordinate medical transportation to the physician visits. In addition, the RNCM educated the member on the purpose and importance of adhering to his medical regimen.  The member subsequently kept his appointment and was grateful for the assistance.



During a recent visit with a member, our RN Care Manager discovered the member was not taking his medications as ordered by the provider. The member admitted he had difficulty swallowing his pills. The RNCM coached the member on how to prepare the medications by mixing them with certain foods like applesauce and/or pudding to make them easier to swallow. Subsequently, the member reported being adherent with the medications during the follow-up call.



Our RN Care Manager received a telephone call from the wife of member, who has been working with the RNCM on his diet and exercise, to get his blood sugar under control. She was very excited, because her husband’s recent A1c was now normal at 6.1. The wife reported that the doctor was very happy and reinforced the diet and lifestyle changes that the member and his wife had made. The member’s wife expressed her gratitude to the RNCM for working so closely with her and for providing her with thorough education and materials which were easy for her to understand.



A member with asthma, overweight and history of back pain and surgery, has been active in the health management program for several months. The member was resistant to lifestyle changes but agreed to work with the RNCM. After many contacts and coaching, the member developed a care plan with the RNCM to lose weight. Subsequently, the member committed to routine exercise and improved his eating habits; the member lost 28 lbs and is motivated to continue with the exercise and weight loss.



A member with diabetes, abnormal blood glucose level and overweight admitted to poor eating habits and exercise. The RNCM provided education on disease process and risks. The member was coached on exercise, nutrition related to diabetes and normal cholesterol levels. Subsequently, the member successfully made diet modifications and started an exercise regimen. The initial A1c level was abnormally high on enrollment to the Program and is now within normal limits; the Cholesterol level (LDL) was abnormally high and is now within normal range.
