City of Tempe, Arizona Extends Contract with U.S. Preventive Medicine for Employee Health Management Program
U.S. Preventive Medicine, the leader in disease prevention, announced today that the City of Tempe, Arizona has extended its contract to provide disease management services to employees who have asthma or diabetes and are covered under the City’s PPO Medical Plan.
JACKSONVILLE, FL, August 1, 2008 — U.S. Preventive Medicine, the leader in disease prevention, announced today that the City of Tempe, Arizona has extended its contract to provide disease management services to employees who have asthma or diabetes and are covered under the City’s PPO Medical Plan. Under the new contract, employees with evidence of cardiovascular disease including hypertension and hyperlipidemia will also be eligible to participate.
The program, Experience Health365…Tempe, began in July 2007. U.S. Preventive Medicine, through its wholly-owned subsidiary, U.S. Care Management, uses The Prevention Plan CM, its highly-regarded field-based chronic disease management program, to manage the program.
Under the Experience Health365 program, U.S. Care Management provides registered nurse care managers who help members manage their chronic diseases, 24/7 call center support, health risk appraisals, and education and information materials. As part of their services, nurses work with members to develop a care plan, make sure members receive timely and proper medical care for their condition and assist the members in understanding and adhering to their doctor’s treatment plan. The RN care managers also monitor the members’ condition, remind them about the importance of taking their medication, and educate them about how to live healthy lifestyles.
The City of Tempe recognizes that chronic health conditions like asthma, diabetes and cardiovascular disease can impact the quality of life and productivity of its employees.
