According to the Centers for Disease Control, nearly one-third of Americans (90 million) suffer the physical, emotional, psychological and financial hardships of chronic disease. Patients with diabetes, asthma, congestive heart failure, hypertension and other chronic diseases use a disproportionate share of medical resources, often receiving care from multiple providers.
