BirthWait Logo High-risk pregnancies and pre-term deliveries are the result of multiple factors including medical, social and economic issues. “BirthWait® contributes to solutions that seek to improve birth outcomes and reduce costs associated with complicated pregnancies and at-risk infants by focusing on the needs of the whole person.

U.S. Care Management utilizes its unique community-based approach to identify women who are likely to have a high-risk pregnancy and at-risk infants up to one year of age.  Women enrolled in the program are each assigned a registered nurse who provides care coordination, assessments, individualized education and referral services through home visits and a 24-hour, seven-days-a-week nurse phone line including in and out-bound calls.

BirthWait® nurses utilize a case management database system, Casewatch® that includes guidelines and indicators for high-risk pregnancy as well as other chronic conditions the mother may have including diabetes, hypertension, asthma and others.  The system offers the same capabilities for at-risk infants. 

The program works closely with medical and community based providers. Obstetrical evidence-based practices are used as the platform for care coordination, patient education and support.
“She (USCM nurse care manager) helped me a lot. I have a very bad sight, but I had to take my insulin injection. That was a problem because I took too little or too much, or took it with some air. Rosie (USCM nurse care manager) called for me and I started receiving ready to use syringes with insulin. I needed to visit an eye doctor, but did not know where to go. Rosie called for me to all hospitals and made an appointment with a good doctor.”
